We’re Looking for You
We are seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate delivers high-quality clinical care with a strong emphasis on hospitality and alignment with our mission, vision, and values. In this role, you will support patient care across the clinical journey, collaborate closely with Nurse Practitioners, Registered Nurses, and providers, and play a vital role in delivering our comprehensive longevity care model. A robust onboarding and training program will be provided to set you up for success at Mantality Health.

What You’ll Do
Perform patient intake, clinical assessments, and vital sign collection
Support providers during patient visits and procedures
Administer injections and therapies as ordered (including hormone therapy and wellness treatments) within LPN scope of practice
Perform phlebotomy and properly handle and process lab specimens
Educate patients on treatment plans, lab testing processes, and services offered under provider direction
Monitor patient status and report changes or concerns to the provider or RN team
Support new and returning patients throughout their care journey
Maintain accurate, timely, and compliant patient documentation in the EMR
Collaborate with Nurse Practitioner, Phlebotomist or Certified Medical Assistant, and administrative staff to ensure seamless patient care
What You Have
Active Iowa Licensed Practical Nurse (LPN) license
Minimum 1–3 years of clinical or wellness experience (longevity, hormone therapy, or similar preferred)
Strong clinical skills and attention to detail
Proficiency in phlebotomy techniques
Excellent interpersonal and communication skills
Ability to work effectively under the direction of the Nurse Practitioner and Clinic Manager and as part of a collaborative care team
Commitment to ongoing learning and best practices in longevity and preventative care

$60,000 – $70,000 annual salary
